An IOL that spontaneously developed opacites 15 years after cataract surgery. No classic reason why this happened (no history of air or gas, no asteroid, no silicone oil in eye). Referring MD tried to treat with yag. Ended up doing an IOL exchange. Pathology says it looks like calcium. Discussion points: causes of IOL opacification, IOL exchange tips, challenged with IOL exchange in a eye that's had a Yag capsultomy.

Hyper mature lens from trauma that needed miLOOP removal with phaco anterior vitrectomy and cerclage. Discussion points: strategy for cataract surgery in dense lenses, miLoop, iris cerclage techniques, through process for anterior segment reconstruction.
