A 67-year-old female with narrow angles status post bilateral laser peripheral iridotomy presents for cataract evaluation and adamantly desires spectacle independence. Pre-operative evaluation reveals irregular astigmatism OD>>OS. The ectasia OS was felt to be mild enough to proceed with trifocal IOL. Cataract surgery was uneventful. Post operative refraction at post-op month one was slightly hyperopic, with uncorrected visual acuity of 20/25 distance. She is happy with the outcome, but desires better near vision and is persistent in requesting spectacle independence. The IOL master and topography suggest possible forme fruste keratoconus. How would you proceed with the fellow eye?
