A 40-year-old patient with a history of phakic iris-claw lens presented for progressive vision loss in the right eye. On examination, we found an iris-claw lens and an underlying subcapsular cataract. A scleral tunnel was fashioned followed by disenclavation of the rigid iris-claw lens. The cataract was extracted with phacoaspiration only and a foldable intraocular lens was implanted in the bag.
