This was a case with a large PCR in the presence of retained large nucleus fragments. I used the three-piece IOL as a scaffold to safely emulsify nuclear fragments without the risk of fragment drop. Once the IOL was placed in the sulcus, the IOL optic was buttoned on the anterior capsule to secure a tight compartmentalization. A limbal vitrectomy was carried out to ensure no vitreous was left.
