A 45-year-old butcher suffered penetrating ocular trauma with a knife. A corneoscleral wound closure, lateral rectus exploration, lensectomy and pars plana vitrectomy was performed guided by intraoperative OCT. Intraoperative examination suggest endophthalmitis and nasal retinal tear. Anterior segment OCT help us identify the type of wound and reduce corneal steps and improve wound healing.
