Uveitis-Glaucoma-Hyphema (UGH) syndrome is often caused by the optic or haptics of one-piece intraocular lenses (IOL) in the sulcus space. We describe a novel cause of UGH in a patient with pseudo-exfoliation and in-the-bag IOL, with extensive fibrosis and phimosis of the anterior capsule and secondary ciliary body traction. Our management included endoscopic guided debridement of this fibrosis.
