A staged strategy for misdirection syndrome in cataract surgery is outlined. Starting with a Honan balloon or osmotic agents, progressing to aqueous drainage by sclerotomy or anterior vitrectomy, and, in severe cases, posterior vitrectomy with hyaloidectomy, double capsulectomy, and iridectomy. This protocol aims to optimize outcomes in complex surgical scenarios.
